Where is the Academic Affairs Office located?

Our office is located at 5000 Hennessy Boulevard, Unit 2 South, Rooms 2242-2247, Baton Rouge, LA, 70808. Take Elevator E to the 2nd Floor, Unit 2 South is directly to the left.

What are the hours of operation for Academic Affairs?

Our hours of operation are Monday through Friday, 8:00 a.m. to 4:00 p.m. except holidays.

I’m interested in rotating at Our Lady of the Lake. How do I do this?

You will first need to contact your program coordinator to ensure the program and school have an active affiliation agreement with Our Lady of the Lake. Once this is confirmed, please have your program coordinator contact Academics@fmolhs.org for next steps.

I’m interested in observing an FMOLHS physician or healthcare provider. How do I do this?

You will need to first connect with a physician or healthcare provider who is willing to serve as your Sponsoring Practitioner. Once this is complete, you can email Academics@fmolhs.org to request an observer application packet.

What is the difference between an observation and a rotation?

Observations do not include hands-on patient experiences. Rotations are reserved for hands-on patient experiences that count towards academic credit.

Where can I find the forms I need to complete in order to start my observation or rotation?

Application packets can be requested by emailing Academics@fmolhs.org.

What is a Sponsoring Practitioner?

A Sponsoring Practitioner is the FMOLHS employee or practitioner overseeing the rotation.

I would like to rotate at Our Lady of the Lake, but I do not have a Sponsoring Practitioner. Do you match learners with Sponsoring Practitioners for rotations?

Academic Affairs does not match learners with practitioners for observations or clinical rotations. Please check with your program coordinator for guidance on obtaining a Sponsoring Practitioner.

What is the Sponsoring Practitioner Letter?

The Sponsoring Practitioner Letter (SPL) gives detailed information about your rotation and who will provide supervision. Where you will complete your rotation, the type of access in EPIC, and the start/end date of the rotation are just a few things listed on the document.

I’m a Health Professional or Non-Clinical Learner attempting to complete the items needed for the Clinical Attestation. Where can I receive ____? (TB skin test, drug screen, background check, etc.)

Learners should check with their program or affiliated university’s Health and Safety office for recommendations on where to receive these requirements.

I have been told I need to complete Epic training, but I am unable to access the virtual training. What should I do?

Epic training is conducted through two systems and learners will need to download both by following the instructions given to ensure access 24 hours before the scheduled training. If you have trouble accessing the training, please contact Academic Affairs.

I’ve already completed Epic training as an FMOLHS employee. Why do I need additional Epic training for my rotation?

You may be required to complete additional Epic training because the access you have as an employee may not align with your clinical rotation.

I’ve already completed Epic training at another institution outside of FMOLHS. Why do I need additional Epic training for my rotation?

FMOLHS has a specific instance of Epic that may not be the same as other health systems.

I have completed Epic training. What do I do now?

You will be contacted by your Sponsoring Practitioner for next steps as you begin your rotation.

Where do I go on the first day of my rotation?

Please contact your Sponsoring Practitioner for information about where to go on the first day of your rotation.

I’m rotating at Our Lady of the Lake Regional Medical Center or Our Lady of the Lake Children’s Hospital. Where do I park for my rotation?

Please see the provided orientation materials provided by Academic Affairs.

Where and when can I pick up my ID badge?

You may pick up your ID badge once you are notified via email that it is ready at the Academic Affairs office. We are located at 5000 Hennessy Boulevard, Unit 2 South, Rooms 2242-2247, Baton Rouge, LA, 70808. Take Elevator E to the 2nd Floor, Unit 2 South is directly to the left.

I have misplaced or lost my ID badge. How do I get a new one?

If you lose or misplace your ID badge, Academic Affairs can provide a replacement in our office upon request. Replacement badges are subject to a $10 replacement fee (cash only). You will need to bring a valid driver's license with you for identification purposes. Please contact academics@fmolhs.org to let us know you need a replacement ID badge. It can take up to fifteen minutes to complete the process for replacing your badge, so please let us know in advance when you will be coming.

I’m needing to e-prescribe controlled substances through Imprivata on my mobile device. What do I need to do to get this activated?

First, you will need to complete Epic training. Once completed, you will need to come to the Academic Affairs office to enroll your mobile device. Please be sure to bring a valid driver's license and the mobile device you would like to enroll.

Imprivata has stopped working on my mobile device. Who do I contact for assistance?

When this occurs, Academic Affairs must re-enroll your device in our office. Make sure you download the app on the mobile device you will use and bring a valid driver's license.

I forgot my password. How do I get it reset?

Contact the IS Support Center at 225-765-HELP (4357) to reset your password.
