St. Clare Manor Admission Process

Each resident at St. Clare Manor must be admitted by a licensed physician and remain under a physician's supervision.

Applicants for admission are assessed by one of our professional staff to determine if their needs can be met by St. Clare Manor. A potential resident's current primary care physician will provide clinical information before admit. These items include and are only good for 30 days:

  • Signed and dated statement of medical status
  • Evidence of TB screening
  • Current physician orders
  • A history and physical

The resident or sponsor completes an admissions packet and provides us with copies of a living will (if in existence), power of attorney (if in existence), and copies of the front and back of Medicare and insurance cards. Everything will be turned in to the admissions department and reviewed in advance of your desired admission date.
