The annual membership fee is $2,000 payable as a lump sum or $2,100 payable in equal monthly installments of $175. The fee/monthly installments will be collected via automatic payments from your debit card, credit card, or checking account. If your company provides this service for you, please provide the contact information, and we will invoice your company.

Certainly. However, membership will be closed once we reach 600 members. At that time, we will establish a waiting list and take new members on a first-come, first-served basis.

Since you will require fewer trips to our office, it may be advantageous to consider increasing your annual deductible, thereby reducing your monthly premiums in order to help offset your membership fee. Additionally, Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA) can be used to offset the cost of any “qualified expense” not reimbursed by your insurance company. If your medical issue requires a visit to our office, we will bill your insurance company for the visit. You will be responsible for any coinsurance or copay that is required under contract by your insurance company.

Insurance companies will not cover your annual membership fee.

Since the ultimate goal is to have more time with our patients during each visit, we will limit the number of members to 600.

While we currently do not offer family memberships, we have options that cater to other members within your family. 

  • Our Junior Membership is for males between the ages of 16 and 30, and costs $1,000 annually.
  • If you are a Platinum member, you are able to add your spouse to your membership for an additional $8,000 per year.

While our Men's Health memberships are designed for men only, Platinum members are able to add their spouse to their membership pan for an additional $8,000 annually.

Yes, we offer a Junior Membership for males between the ages of 16 to 30 years old that costs $1,000 annually.

Men ages 16 and older are eligible.

If you would rather continue to have the more comprehensive Executive Wellness Exam that you are accustomed to, that will not be a problem. You will simply upgrade your membership for an additional fee.

As a patient of Lake Men's Health, you will see either Dr. Curtis Chastain or Dr. Tyler Boudreaux, depending on their availability and your type of visit. 

You can request an appointment or ask a question through text message, and we’ll get back to you quickly. As texting is not secure, we do not recommend sending detailed medical questions by text.

Absolutely. If you paid a lump sum for your enrollment and have not had your Annual Health Review, we will gladly refund a prorated amount of your membership fee.

We have a designated office cell phone that you will have unrestricted access to call or text during business hours.

If a situation arises after hours, please call the office and you will be directed to our answering service. They will assess your issue and contact one of us if necessary.

If you require hospitalization, you will be admitted by one of our physician colleagues. He/she will manage your care during your entire hospital stay and will be in direct and constant communication with us regarding your care.

Our physician colleagues in the emergency room will see you and contact us if necessary.

If your medical issue requires a referral to a specialist, we will make a recommendation for you and help facilitate your appointment. However, since you know your schedule best, it’s optimal for you to make your appointment.
