PACE Baton Rouge (Program of All-Inclusive Care for the Elderly) is a nonprofit health and supportive services program designed to assist seniors who want an alternative to long-term care placement. PACE is sponsored by the Franciscan Missionaries of Our Lady Health System (FMOLHS)-the parent company of Our Lady of the Lake Regional Medical Center-the Centers for Medicaid and Medicare Services (CMS) and the Louisiana Department of Health and Hospitals (DHH).

About PACE Baton Rouge
The mission of PACE Baton Rouge is to enable the aging population to live with dignity in their communities. With PACE Baton Rouge's assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance; physical relief and emotional support.
PACE Baton Rouge takes a team approach to care which revolves around its diverse, multi-disciplinary team. The team works together to develop individual care plans, goals and carefully monitors progress.

Admissions Process
The PACE enrollment process includes a screening by phone to introduce the program and determine eligibility, and a health assessment to determine participant's needs. The candidate must meet the eligibility requirements set forth by DHH and CMS. The enrollment process includes a phone interview to answer any questions about the program or cost.
