What information should I bring with me when I am registering for an inpatient or outpatient admission?

I cannot afford to pay for services. Is assistance available?

As a Catholic healthcare facility, Our Lady of the Lake Ascension is committed to meeting the health needs of God's people with compassion, understanding, respect and dignity. The hospital acknowledges its responsibility to provide healthcare-related services to the community and especially to those who are unable to pay for these services.

If you do not have health insurance, our staff may help you find government or private programs that may assist you.
If you are uninsured or do not qualify for a government sponsored insurance program, you may be eligible for free or discounted services through Our Lady of the Lake Ascension Our Lady of the Lake’s Financial Assistance Policy depending on your income level and circumstances. Depending upon eligibility, you may also be offered payment arrangements that are scheduled over time.

Where can I donate blood for myself or for someone else?
Our Lady of the Lake Ascension operates a Blood Donor Center located at 1404 W. St. Clare Boulevard in Gonzales. Hours of operation are: Monday-Thursday, 8 a.m. - 6 p.m. , Friday, 8 a.m. - 4:30 p.m.

Can I fill required paperwork before arriving at the hospital?
At this time, all necessary paperwork must be filled out upon arrival at the hospital.

Where do I call to help me find a doctor nearby?
You can find a physician using our Find a Doctor database.

Where do I call if I have a question about my hospital bill?
Call Billing Services at (225) 621-2932.

What is the Health Information Management Department?
This is the name hospitals use to designate the department that holds your medical records.

How can I get a copy of my medical records for myself or have them sent to someone other than myself, such as a physician, attorney or insurance company?
To obtain copies of your medical records, you will be asked to complete an Authorization for Release of Health Information form. Clearly specify the dates of service and/or the types of reports needed. Records will not be released without verification of a valid picture ID.

All patient health care information at Our Lady of the Lake Ascension is confidential and protected by State and Federal laws and HIPAA regulations. A patient, or his/her legal representative, may inspect and/or obtain a copy of their medical record, or have copies of medical records sent to another facility.

Our Lady of the Lake Ascension requires a completed, signed and dated Authorization for Release of Health Information form before releasing documents to anyone, including patient. Because a signature is required to release your information, we are not able to process requests received by telephone or email. When you pick up your medical records in person, we will ask for a photo identification. There maybe a fee for providing copies of the medical record. Can I get my lab results over the phone?
The physician can release the lab results over the phone if the results are being made available as part of a visit or treatment episode. All other requests for lab records should be directed to the Health Information Management Department.

How do I obtain a copy of my Radiology Films?
To obtain copies of your radiology films, you will be asked to complete an Authorization for Release of Health Information form. Clearly specify the dates of service. To avoid extended wait times, you can call the Radiology Department to request films in advance at (225) 621-2990. Films will not be released without verification of a valid picture ID.
